A dedicated Registered Nurse (RN) with strong Home Health and OASIS experience is needed for a 13-week full-time contract in home health. Schedule:
- 13-week contract with potential to extend
- Full-time hours, Monday through Friday
- On-call availability may be required
- Structured daily visit expectations
What You'll Be Doing:
- Deliver skilled in-home nursing care and clinical interventions
- Conduct patient assessments, completing OASIS documentation thoroughly and on time
- Collaborate with physicians and other care professionals to implement individualized care plans
- Perform wound care, medication administration, patient education, and monitor patient health status
- Consistently communicate important updates or changes in patient condition with the clinical team
Qualifications:
- Active RN license (unrestricted)
- At least 1 year of recent RN experience in home health settings
- Demonstrated OASIS documentation proficiency (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Ability to maintain a structured, full-time schedule and meet daily patient visit requirements
- Reliable transportation and valid drivers license
