Provide compassionate end-of-life care as a Hospice Registered Nurse (RN) with a respected healthcare agency in Courtland, MS. This contract opportunity allows you to support patients and their families through one of lifes most important phases, delivering expert nursing care in a home health setting.
Key Responsibilities:
- Conduct thorough patient assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure holistic care delivery
- Provide symptom management and emotional support to patients and families
- Maintain accurate and timely documentation using healthcare software, preferably HCHB
- Educate patients and families on care options and resources available
Qualifications:
- Valid RN license in good standing
- Minimum of 2 years experience in hospice or palliative care nursing
- Proficiency with Hospice Home Care Homebase (HCHB) software is preferred
- Excellent clinical assessment capabilities and communication skills
- Demonstrated compassion and empathy in patient care
- Reliable transportation with a valid drivers license to travel to patient homes
