Thrive in an impactful field-based nursing role with this full-time, 13-week contract opportunity serving home health patients. You'll work Monday through Friday, 8:00 AM to 5:00 PM, managing a caseload of approximately 30 points per weektypically 3 to 5 patient visits daily. This position is ideal for a Registered Nurse who excels at independent patient care, collaborative communication, and making a difference directly in peoples homes.

Key details and schedule:

• Full-time, 13-week contract
• MondayFriday, 8:00 AM 5:00 PM
• Field-based; daily travel to patient homes within assigned territory (mileage reimbursement provided)
• Caseload: 35 patient visits/day, 30 points/week

Qualifications:

• Active, unrestricted RN license in the state of practice
• Minimum 1 year of recent home health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Strong OASIS assessment skills
• Reliable transportation, valid drivers license, and current auto insurance

Responsibilities:

• Perform thorough OASIS assessments and develop tailored care plans
• Deliver skilled nursing services, including wound care, IV therapy, chronic disease management, and medication administration
• Monitor patient status, communicate changes promptly, and ensure interdisciplinary care coordination
• Educate patients and families about disease processes, treatment, medications, and prevention strategies
• Accurately maintain electronic documentation using HCHB
• Collaborate with therapists, medical social workers, and home health aides
• Participate in case conferences, staff meetings, and ongoing education

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
