Schedule/Hours:

• Weekdays, MondayFriday, 8:00 a.m.5:00 p.m.

Qualifications:

• Minimum of 1 years RN experience in home health (required)
• Valid RN license
• Valid drivers license and proof of insurance
• BLS certification
• Proficiency with Homecare Homebase (HCHB) and OASIS documentation (required)
• Strong skills in wound care, wound vacs, IV infusions, PICC/port access, drain management, catheter insertion, and infection control
• Ability to drive at night
• Open to first-time travelers (if home health RN background is present)

Responsibilities:

• Coordinate and schedule daily patient visits (67 units/day; 30/week)
• Manage diverse adult patient caseloads (age 18+)
• Provide skilled nursing for wound care, IVs, medication and disease management, catheter/drain care, and patient/caregiver education
• Collaborate with LPNs, CNAs, department leadership, and case management
• Document care using Homecare Homebase on a provided Samsung Note 9 device
