Take your nursing expertise to the next level by delivering compassionate, skilled care to patients in their homes. This full-time, 13-week contract role offers the chance to provide high-quality, patient-centered care in a field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

In this position, you'll manage a caseload with an expectation of reaching approximately 30 points per week, typically involving 35 patient visits daily within a defined territory. Travel is required, and mileage reimbursement is provided.

Key Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop detailed, individualized care plans.
• Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and document patient progress, promptly communicating any changes to the interdisciplinary care team and physicians.
• Educate patients and families on diagnoses, medications, and care plans to promote optimal outcomes and independence.
• Maintain accurate, timely records using the Home Care Home Base (HCHB) system.
• Collaborate with therapists, medical social workers, and home health aides to ensure coordinated, seamless care delivery.
• Engage actively in case conferences, staff meetings, and ongoing professional development.
• Consistently adhere to all agency and regulatory standards in patient care and documentation.

Qualifications:

• Active RN license in good standing in the practice state
• Minimum 1 year of recent home health experience (field experience highly preferred)
• Demonstrated proficiency with Home Care Home Base (HCHB)
• OASIS experience required
• Reliable transportation, valid drivers license, and current auto insurance

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
