Your next nursing opportunity starts here with a rewarding full-time home health assignment in Converse, TX. We are seeking a dedicated Registered Nurse with proven OASIS experience to deliver outstanding care as part of a professional and supportive clinical team. This 13-week contract offers a chance to build meaningful connections with patients while advancing your clinical expertise.
Schedule:
- Monday through Friday, full-time hours
- On-call coverage likely as part of rotation
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing interventions, ensuring each patient receives personalized care
- Accurately complete patient assessments and OASIS documentation in a timely manner
- Collaborate closely with physicians and multidisciplinary professionals to develop and adjust individualized care plans
- Provide medication management, wound care, disease monitoring, and patient/family education
- Clearly communicate updates and changes in condition to the entire clinical team
Qualifications:
- Active and unrestricted RN license
- At least one year of recent experience as a Home Health RN
- OASIS competency is required
- Proficiency with HomeCare HomeBase strongly preferred
- Demonstrated ability to manage a full-time structured schedule involving daily patient visits
- Reliable vehicle and current drivers license
