We are seeking a dedicated Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week, full-time contract assignment. Schedule:
- 13-week full-time contract
- Monday through Friday
- On-call duties may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to patients
- Complete thorough patient assessments and timely, accurate OASIS documentation
- Work closely with physicians and other healthcare providers to tailor individualized care plans
- Provide wound care, manage medications, educate patients, and monitor chronic conditions
- Communicate any changes or updates in patient conditions to the full clinical team
Qualifications:
- Current, active RN license
- At least one year of recent home health RN experience
- Required proficiency in OASIS documentation
- Familiarity with HomeCare HomeBase preferred
- Capable of managing a consistent, full-time schedule including daily patient visits
- Must have reliable transportation and a valid drivers license
