A skilled Registered Nurse (RN) with a background in home health is needed for a full-time, 13-week contract assignment. This role focuses on in-home patient care, accurate documentation, and collaboration with a multidisciplinary team. Schedule:

• Full-time, Monday through Friday
• On-call duties may be required

Key Responsibilities:

• Deliver direct, in-home nursing care and skilled interventions
• Conduct patient assessments and complete OASIS documentation accurately and timely
• Coordinate and collaborate with physicians and care teams to deliver individualized care plans
• Perform wound care, medication administration, patient education, and ongoing disease monitoring
• Clearly communicate patient updates or changes in condition to the broader clinical team

Qualifications:

• Active RN license
• Minimum of 1 year recent RN experience in home health
• OASIS documentation experience is required
• Familiarity with HomeCare HomeBase is strongly preferred
• Strong organizational skills with ability to meet daily patient visit expectations
• Reliable transportation and an active drivers license
