A skilled Registered Nurse (RN) with a background in home health is needed for a full-time, 13-week contract assignment. This role focuses on in-home patient care, accurate documentation, and collaboration with a multidisciplinary team. If you excel at managing a busy schedule and delivering clinical excellence, you'll thrive in this assignment.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver direct, in-home nursing care and skilled interventions
- Conduct patient assessments and complete OASIS documentation accurately and timely
- Coordinate and collaborate with physicians and care teams to deliver individualized care plans
- Perform wound care, medication administration, patient education, and ongoing disease monitoring
- Clearly communicate patient updates or changes in condition to the broader clinical team
Qualifications:
- Active RN license
- Minimum of 1 year recent RN experience in home health
- OASIS documentation experience is required
- Familiarity with HomeCare HomeBase is strongly preferred
- Strong organizational skills with ability to meet daily patient visit expectations
- Reliable transportation and an active drivers license
