We are seeking a dedicated Registered Nurse (RN) with proven OASIS experience for a 13-week full-time contract serving patients in their homes. This role is ideal for nurses who are confident in clinical decision-making, excel at accurate documentation, and thrive with structured daily schedules.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
- 13-week contract (with potential for extension)
Key Responsibilities:
- Deliver skilled in-home nursing care based on individual patient needs
- Perform comprehensive assessments and complete OASIS documentation accurately and within required timeframes
- Collaborate closely with physicians and multidisciplinary care teams to implement tailored care plans
- Provide wound care, manage medications, educate patients and families, and monitor disease processes
- Clearly communicate updates and changes in patient conditions to the clinical team
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience in home health nursing
- Required expertise with OASIS documentation
- Strongly preferred: Proficiency in HomeCare HomeBase software
- Ability to manage a full-time, structured visit schedule independently
- Reliable transportation and a valid drivers license
