This opportunity is for an experienced Registered Nurse seeking a 13-week full-time contract. Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- Structured daily patient visits; on-call participation probable
Key Responsibilities:
- Deliver skilled nursing care and interventions within patient homes
- Conduct comprehensive patient assessments and complete OASIS documentation accurately and timely
- Collaborate with physicians and care teams to develop individualized care plans
- Perform wound care, medication management, patient and caregiver education, and monitor disease progression
- Communicate changes in patient conditions and updates to the clinical team
Who Were Looking For:
- Current, active RN license
- Minimum of 1 year recent experience as a home health RN
- Proven OASIS documentation skills
- Experience with HomeCare HomeBase software strongly preferred
- Capable of managing a consistent, full-time schedule with daily visits
- Possess reliable transportation and a valid drivers license
