Deliver compassionate, high-quality care to patients in a rewarding field-based nursing role focused on home health. This opportunity offers a 13-week full-time contract, providing vital services directly in patient homes and allowing you to make a meaningful impact in your community.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Your Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient conditions, recognize and report any changes, and collaborate closely with the care team and physicians.
• Educate patients and their caregivers on treatment plans, medication regimens, and preventive care strategies.
• Accurately complete all documentation in Home Care Home Base (HCHB).
• Work collaboratively with physical therapists, occupational therapists, social workers, and home health aides.
• Participate actively in case conferences, staff meetings, and ongoing professional development.
• Adhere to established documentation and patient care standards.

Requirements:

• Active, unrestricted RN license in the state of practice.
• At least one year of recent home health nursing experience (field experience preferred).
• Demonstrated proficiency with Home Care Home Base (HCHB) and experience working with OASIS assessments.
• Dependable transportation, a current drivers license, and up-to-date auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
