Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required
• Daily visit structure for consistent workflow

Key Responsibilities:

• Deliver in-home nursing care and skilled interventions to a diverse patient population
• Perform thorough assessments and complete OASIS documentation accurately and promptly
• Develop individualized care plans in collaboration with physicians and interdisciplinary teams
• Provide wound care, manage medications, educate patients, and monitor disease progression
• Effectively communicate patient updates and any changes in condition to the clinical team

Qualifications:

• Current RN license in good standing
• At least 1 year of recent experience as a home health RN
• Demonstrated experience with OASIS documentation
• Experience with HomeCare HomeBase is strongly preferred
• Confidence in managing a full-time, structured home health schedule
• Possess a valid drivers license and reliable transportation

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
