Take your nursing expertise into the community as a Home Health Registered Nurse, providing vital care to patients in their own homes. This full-time, 13-week contract position offers the chance to make a significant impact while working independently and collaboratively with an interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Caseload and Setting:

• Average 30 points per week (typically 35 patient visits daily)
• Field-based, with daily travel to patient homes within your assigned territory

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
• Track patient progress, identify changes in condition, and update the care team promptly
• Provide education to patients and families on disease processes, care plans, and preventive practices
• Ensure timely and accurate documentation using Home Care Home Base (HCHB)
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides for coordinated patient care
• Participate in case conferences, staff meetings, and ongoing education sessions
• Uphold all regulatory and agency standards for documentation and patient care

Job Requirements:

• Current, active RN license in good standing for the state of practice
• At least one year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB)
• Experience completing OASIS assessments
• Dependable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
