Deliver compassionate, skilled care directly to patients homes as part of a dynamic field-based nursing team. This full-time, 13-week contract position offers the chance to work one-on-one with patients, leveraging your expertise in home health nursing and proficiency with tools such as OASIS and Home Care Home Base (HCHB).

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Role Highlights:

• Maintain a caseload of approximately 30 points weekly (typically 35 patient visits per day)
• Provide daily travel within your assigned territory (mileage reimbursement included)
• Work exclusively in patient homes to ensure continuity and quality of care

Primary Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services: wound care, medication administration, IV therapies, and management of chronic diseases
• Monitor patient status, identify changes, and promptly update care teams and physicians
• Educate patients and families on treatment protocols, medications, and health maintenance
• Accurately document patient encounters in HCHB in a timely manner
• Collaborate with interdisciplinary team memberstherapists, social workers, and aidesto coordinate effective care
• Attend case conferences and staff meetings, and pursue ongoing education
• Adhere to all clinical and regulatory documentation requirements

Desired Qualifications:

• Active, unencumbered RN license for the state
• Minimum 1 year of recent home health nursing experience (field-based strongly preferred)
• Solid command of HCHB and OASIS documentation
• Reliable vehicle, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
