We are seeking an experienced Registered Nurse (RN) with a solid background in home health and OASIS documentation for a full-time 13-week contract. Schedule & Hours:
- 13-week full-time contract with potential to extend
- Monday through Friday schedule (on-call responsibilities likely)
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home skilled nursing care to diverse patient populations
- Conduct comprehensive patient assessments, completing OASIS documentation accurately and promptly
- Work closely with physicians and multidisciplinary care teams to develop and adjust individualized care plans
- Administer wound care, manage medications, provide disease monitoring, and educate patients and families on health management
- Communicate changes in patient status to clinical team members quickly and efficiently
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Required: Experience with OASIS documentation
- Strong preference for proficiency in HomeCare HomeBase
- Able to manage a structured caseload, meeting daily visit requirements
- Reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
