We are seeking a dedicated Registered Nurse with strong OASIS experience to join a professional and compassionate care team for a 13-week full-time home health contract. If you are confident in your clinical skills, excel in documentation accuracy, and can effectively manage a structured schedule, this position offers a chance to make a meaningful difference in patients lives.
Schedule & Assignment Details:
- 13-week contract, potential for extension
- Full-time, Monday through Friday
- Likely on-call responsibilities
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Perform thorough patient assessments and complete accurate, timely OASIS documentation
- Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans
- Provide wound care, manage medications, educate patients, and monitor disease progression
- Communicate changes in patient conditions to the clinical team promptly
Qualifications:
- Active RN license (required)
- At least 1 year of recent experience in home health as an RN
- OASIS documentation expertise (required)
- Proficiency with HomeCare HomeBase strongly preferred
- Ability to maintain a structured daily visit schedule
- Reliable transportation and a valid drivers license
