Your next step in nursing starts with a meaningful home health opportunity. We are seeking an experienced Registered Nurse (RN) with recent OASIS expertise for a 13-week full-time contract. This role suits a self-driven clinician ready to ensure quality care and excellence in documentation within home health.

Schedule:

• 13-week contract, potential to extend
• Full-time, Monday through Friday
• On-call participation possible
• Daily, structured patient visit requirements

Qualifications:

• Current and active RN license
• At least 1 year of recent home health experience as an RN
• Strong proficiency with OASIS documentation
• HomeCare HomeBase experience preferred
• Proven ability to manage a structured schedule and daily patient assignments
• Reliable transportation and valid drivers license

Key Responsibilities:

• Deliver compassionate nursing care in patients homes, including skilled and direct interventions
• Complete accurate, timely OASIS assessments for each patient visit
• Partner with physicians and interdisciplinary teams to develop and implement individualized care plans
• Provide wound management, medication administration, disease monitoring, and patient/caregiver education
• Promptly communicate patient status updates and any changes in condition to the clinical care team
