Advance your nursing career in a dynamic field-based role providing essential patient care in home settings. This full-time, 13-week contract offers the opportunity to make a significant impact through individualized care and skilled nursing services.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct thorough OASIS assessments and develop care plans tailored to each patients needs.
• Deliver skilled nursing care, including wound and medication management, IV therapy, and chronic disease oversight.
• Monitor patient condition, report changes to the care team, and communicate with physicians for coordinated interventions.
• Educate patients and families on diagnoses, medications, treatment protocols, and preventive health practices.
• Maintain precise and prompt electronic documentation using Home Care Home Base (HCHB).
• Collaborate effectively with interdisciplinary team members, including therapists and social workers.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Ensure compliance with all documentation and patient care guidelines.

Qualifications:

• Active RN license in good standing, valid for the state of practice
• At least one year of recent home health nursing experience (field setting experience preferred)
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Demonstrated experience with OASIS assessments and patient care
• Reliable transportation, a valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
