Job Description
Make a direct impact on patient outcomes by delivering compassionate, high-quality care in a home health setting. This full-time, 13-week contract position is designed for a skilled Registered Nurse who thrives in both clinical autonomy and close patient interaction.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, providing care in patient homes
- Caseload: 30 points per week (typically 35 visits per day)
- Daily travel within assigned territory (mileage reimbursement offered)
What Youll Do:
- Perform comprehensive OASIS assessments and develop personalized care plans
- Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient condition and collaborate with the interdisciplinary care team to ensure optimal outcomes
- Educate patients and families on treatment plans, disease management, and preventive strategies
- Maintain detailed, timely documentation using Home Care Home Base (HCHB)
- Participate in staff meetings, case conferences, and ongoing professional development
- Ensure compliance with all regulatory and agency standards in care delivery
Key Qualifications:
- Current and active RN license (in good standing) for the state of practice
- At least 1 year of recent home health field experience
- Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS assessments
- Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
