Schedule:
- 13-week contract, full-time
- Monday through Friday
- On-call duties may arise
Qualifications:
- Current, unrestricted RN license
- At least 1 year of recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase strongly preferred
- Reliable transportation and a valid drivers license
Responsibilities:
- Deliver skilled nursing care and interventions to patients in their homes
- Conduct comprehensive patient assessments and document findings in OASIS
- Collaborate with interdisciplinary care teams and physicians for individualized care planning
- Provide wound care, medication management, disease monitoring, and patient education
- Communicate patient status updates and changes to clinical teams promptly

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
