Advance your nursing career by delivering vital care directly within patients homes. This full-time, 13-week contract offers the chance to work with a dynamic team dedicated to high-quality, patient-centered care.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based position with daily travel to patient homes (mileage reimbursement provided)
• Expected caseload: 30 points weekly (approximately 35 patient visits per day)

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, such as wound care, IV therapy, chronic disease management, and medication administration
• Monitor patient progress, identify condition changes, and communicate effectively with interdisciplinary teams and physicians
• Educate patients and families on medical conditions, care plans, medications, and preventive strategies
• Ensure accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with other healthcare professionals, including physical therapists, occupational therapists, and home health aides
• Attend case conferences, staff meetings, and participate in ongoing education
• Uphold regulatory and agency standards for documentation and patient care

Qualifications:

• Active RN license in the state of practice (in good standing)
• Minimum 1 year of recent home health nursing experience (field experience highly preferred)
• Proficient with Home Care Home Base (HCHB) electronic documentation system
• Demonstrated OASIS assessment proficiency
• Reliable transportation, current drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
