Job Description

An opportunity is available for a Registered Nurse (RN) with home health experience to provide compassionate and comprehensive care to patients in their own homes. This contract position calls for a dedicated professional with a strong background in home health nursing and familiarity with OASIS assessments.

Key Responsibilities:

  • Deliver nursing care including initial and ongoing patient assessments (vital signs, medication management, wound care).
  • Develop personalized care plans tailored to patient needs.
  • Educate patients and families about disease management, treatments, and preventive care.
  • Collaborate with physicians, therapists, and healthcare team members to coordinate care.
  • Maintain accurate and timely documentation in electronic medical records (EMR).
  • Monitor patient conditions and promptly report any changes.
  • Ensure compliance with state and federal home health regulations.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 3 months of home health nursing experience using OASIS.
  • Experience with Home Care Home Base (HCHB) software is highly advantageous.
  • Strong clinical skills, including IV therapy, wound care, and medication administration.
  • Proficiency in electronic medical records systems.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and manage time effectively.
  • Valid drivers license and reliable transportation.
