Schedule/Hours:

Qualifications and Skills:
- Active RN license in good standing
- Minimum of one year of recent Home Health nursing experience preferred
- Strong clinical assessment and critical thinking abilities
- Proficient in developing, monitoring, and revising care plans
- Familiarity with electronic health records and mobile charting devices
- Excellent interpersonal and communication skills
- Valid drivers license and reliable transportation, as travel between patient homes is required
- Current BLS certification
Key Responsibilities:
- Conduct in-home patient assessments to develop personalized care plans
- Administer medications, perform wound care, and manage IV therapies as clinically indicated
- Provide patient and family education regarding ongoing health care needs
- Collaborate with physicians, therapists, and other care professionals to ensure optimal patient outcomes
- Monitor patient progress and update care plans as needed
- Maintain accurate and timely documentation compliant with healthcare regulations and standards
