This full-time, 13-week contract offers the opportunity to make a meaningful difference for patients in their own homes, utilizing your expertise and clinical judgment in a supportive environment.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based position with daily travel to patient homes
• Maintain a caseload averaging 35 patient visits per day (30 points/week)

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum of one year of recent home health experience (field preferred)
• Proficiency with Home Care Home Base (HCHB) required
• Solid experience performing and documenting OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management
• Continuously evaluate patient status, promptly report changes, and coordinate with the care team and physicians
• Educate patients and families regarding diagnosis, treatment, medication, and preventive care
• Document all patient interactions timely and accurately in HCHB
• Collaborate with multidisciplinary team members for patient-centered care
• Participate in case conferences, staff meetings, and ongoing education to advance clinical skills
• Uphold regulatory and agency standards in documentation and patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
