This full-time, 13-week contract position enables you to provide compassionate, skilled care to individuals in the comfort of their own homes, helping them recover and thrive with dignity and support.

Schedule & Work Details:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 30 points per week (typically 35 patient visits per day)
• Field-based daily travel within an assigned territory (mileage reimbursement provided)
• Utilizes Home Care Home Base (HCHB) for electronic documentation and OASIS assessment tool

Required Qualifications:

• Active RN license in good standing within the state of practice
• Minimum of one year of recent home health nursing experience with direct field work preferred
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Conduct thorough OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and disease management
• Monitor and evaluate patient status, promptly reporting changes to physicians and care teams
• Provide education and guidance to patients and their families regarding conditions, treatments, and preventive health
• Maintain detailed, timely documentation in HCHB system
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides
• Attend case conferences, staff meetings, and ongoing training to ensure excellent patient outcomes
• Uphold agency and regulatory standards for documentation and care delivery

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
