Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse with strong OASIS experience for a 13-week full-time home health contract. This role is designed for a clinically confident RN who excels at documentation and is comfortable managing a structured daily schedulemaking a real impact in patients' lives.

Schedule:

  • 13-week contract, with potential for extension
  • Full-time hours, Monday through Friday
  • On-call responsibilities may be required
  • Structured daily visit schedule

Qualifications:

  • Active RN license
  • At least 1 year of recent home health RN experience
  • OASIS documentation experience required
  • Proficiency with HomeCare HomeBase (highly preferred)
  • Reliable transportation and a valid drivers license

Responsibilities:

  • Deliver comprehensive in-home nursing care and skilled interventions
  • Complete accurate, timely OASIS assessments and related documentation
  • Collaborate closely with physicians and interdisciplinary care teams to tailor care plans
  • Perform wound care, administer medications, deliver patient education, and support disease management
  • Communicate thoroughly regarding patient condition updates to the clinical team
