If OASIS documentation, patient-centered care, and managing a consistent schedule are in your skillset, this position provides a chance to make a significant difference in patients lives.
Schedule:
- 13-week contract, Monday through Friday
- Full-time hours with structured daily visit counts
- Occasional on-call responsibilities likely
Qualifications:
- Active RN license
- At least 1 year of recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable vehicle and valid drivers license
Responsibilities:
- Deliver compassionate in-home nursing and skilled clinical interventions
- Complete patient assessments and OASIS documentation accurately and punctually
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
- Provide wound care, medication management, patient and family education, and disease monitoring
- Communicate significant changes or updates to the clinical team promptly
