We are seeking a dedicated Registered Nurse (RN) with expertise in home health and strong OASIS experience to join a professional care team on a 13-week full-time contract. Schedule:
- 13-week contract, full-time
- Monday through Friday
- On-call likely required
Preferred Qualifications:
- Active RN license
- Minimum of 1 year of recent RN home health experience
- Proficient in OASIS documentation
- Experience with HomeCare HomeBase software (strongly preferred)
- Valid drivers license and reliable transportation
- Strong organizational skills and a commitment to high-quality patient care
Core Responsibilities:
- Deliver skilled nursing care through in-home visits
- Accurately assess patient needs and complete timely OASIS documentation
- Work collaboratively with physicians and interdisciplinary care teams to create and implement individualized care plans
- Perform wound care, medication management, disease monitoring, and patient/family education
- Update the clinical team promptly regarding changes in patient status
