Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Qualifications:
- Current, active RN license
- At least 1 year of recent experience as a home health RN
- Proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase software (strongly preferred)
- Reliable transportation and valid drivers license
- Excellent organizational skills for daily visit management
Primary Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Conduct thorough assessments and complete accurate, timely OASIS documentation
- Collaborate closely with physicians and multidisciplinary care teams for individualized plans
- Manage wound care, medication administration, patient education, and monitoring of chronic conditions
- Provide timely communication regarding patient care updates and any changes in status
