Advance your nursing career by providing expert care to patients in their homes through a full-time, field-based role focused on comprehensive and compassionate service delivery. This 13-week contract offers a dynamic environment and the opportunity to make a meaningful impact in patients daily lives while collaborating with a multidisciplinary team.

Schedule:

• Monday Friday
• 8:00 AM 5:00 PM

Key Responsibilities:

• Perform in-depth OASIS assessments and develop tailored care plans for patients.
• Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and management of chronic conditions.
• Monitor patient progress, identify important changes, and communicate findings promptly to physicians and the care team.
• Teach patients and their families about disease processes, medication regimens, and preventative care strategies.
• Accurately document patient information and care provided in Home Care Home Base (HCHB).
• Engage in coordinated care with physical and occupational therapists, social workers, and home health aides.
• Participate in regular case conferences, staff meetings, and ongoing professional development opportunities.
• Ensure strict compliance with all documentation and care delivery standards.

Qualifications:

• Active RN license in good standing in the state of practice.
• At least 1 year of recent Home Health nursing experience (field-based preferred).
• Proficient in using Home Care Home Base (HCHB) documentation system.
• Demonstrated experience with OASIS assessments.
• Possession of reliable transportation, a current drivers license, and auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
