Were seeking an experienced Registered Nurse with proven expertise in OASIS documentation to deliver high-quality, individualized care in patients homes. This 13-week assignment offers a structured schedule and the chance to work alongside a collaborative, professional healthcare team.
Schedule:
- Monday through Friday, full-time hours
- Occasional on-call responsibilities
- 13-week contract duration with the possibility for extension
Primary Responsibilities:
- Deliver comprehensive in-home nursing care, supporting patients through skilled interventions
- Complete patient assessments and submit detailed, timely OASIS documentation
- Partner with physicians and interdisciplinary care teams to execute effective, personalized care plans
- Provide wound care, medication administration, patient education, and chronic disease monitoring
- Offer clear and consistent communication regarding changes in patient condition to the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of one year of recent RN experience in home health settings
- Demonstrated proficiency with OASIS documentation (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Ability to coordinate a full-time schedule with daily in-home visits
- Valid drivers license and reliable transportation
