An exciting chance awaits an experienced Registered Nurse (RN) with a passion for home health to deliver critical care to patients in their own environment. We are seeking an RN with proven OASIS experience for a 13-week, full-time contractan outstanding way to make an immediate and tangible impact through compassionate, hands-on care.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- On-call likely required in addition to regular weekday hours
- Structured patient visit expectations each day
Key Responsibilities:
- Deliver in-home nursing services and utilize skilled clinical interventions
- Complete detailed patient assessments with precise OASIS documentation, adhering to deadlines
- Partner with physicians and support teams to establish and carry out personalized care plans
- Perform wound management, medication administration, patient teaching, and disease monitoring
- Report patient conditions and care updates to the multidisciplinary clinical team
Qualifications & Desired Experience:
- Current, unrestricted RN license
- At least 1 year of recent experience as a home health RN
- Demonstrated expertise in OASIS documentation
- Familiarity with HomeCare HomeBase (highly preferred)
- Strong organizational skills and reliability with structured scheduling
- Valid drivers license and dependable transportation
