Key Responsibilities:

- Perform wound care, manage medications, provide patient education, and monitor disease progression
- Communicate patient status updates to the clinical care team effectively
Qualifications:
- Active RN license required
- Minimum of one year recent experience as a home health RN
- Proven experience with OASIS documentation is essential
- Familiarity with HomeCare HomeBase is highly preferred
- Ability to manage a full-time structured schedule with regular patient visits
- Reliable transportation and valid drivers license
Schedule:
- Full-time, Monday through Friday with probable on-call duties
