Key Responsibilities:
- Provide comprehensive in-home nursing care, including skilled interventions and patient education

- Collaborate effectively with physicians and care teams to create and adjust individualized care plans
- Manage wound care, medication administration, disease monitoring, and other clinical duties
- Communicate regularly with the clinical team regarding patient status and care updates
Qualifications and Experience:
- Active Registered Nurse (RN) license
- Minimum of one year recent experience in home health nursing
- Proficient in OASIS documentation is mandatory
- Familiarity with HomeCare HomeBase strongly preferred
- Ability to manage a structured Monday through Friday schedule with daily patient visits
- Reliable transportation and a valid drivers license
