Schedule:
- 13-week contract with extension potential
- Full-time, Monday through Friday
- Likely on-call requirements
Key Responsibilities:
- Deliver skilled in-home nursing care to patients
- Complete comprehensive assessments and OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Administer wound care, manage medications, provide patient education, and perform disease monitoring
- Report changes in patient condition promptly to the care team
Qualifications:
- Active RN license
- At least one year of recent home health experience as an RN
- Demonstrated experience with OASIS documentation (required)
- Proficiency with HomeCare HomeBase (highly preferred)
- Reliable transportation and a valid drivers license
- Comfortable managing daily patient visits and a structured schedule
