Experience a dynamic nursing role providing meaningful care directly in patient homes. This 13-week, full-time contract opportunity offers a Monday through Friday schedule from 8:00 AM to 5:00 PM. Youll manage a caseload averaging 30 points per week, typically involving three to five patient visits each day, with daily travel within your assigned areamileage reimbursement is included. The position uses Home Care Home Base (HCHB) for documentation and the OASIS assessment tool to ensure high standards of care.

Key Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans for patients in their homes
• Deliver comprehensive nursing services such as wound care, medication management, IV therapy, and chronic disease intervention
• Monitor patient conditions, communicate changes urgently to the care team and physicians, and revise care strategies accordingly
• Educate patients and families about disease processes, prescribed treatments, medications, and health maintenance
• Document patient encounters and care accurately and efficiently using Home Care Home Base (HCHB)
• Foster collaboration with physical therapists, occupational therapists, medical social workers, and home health aides
• Participate in case conferences, staff meetings, and ongoing professional development
• Maintain compliance with all regulatory standards and best practices for patient care

Required Qualifications:

• Active and unencumbered RN license for the state
• At least one year of recent field-based Home Health experience
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Reliable car, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
