Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Role Responsibilities:

• Visit patients in their homes daily to conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly communicating any changes or concerns to the care team and physicians.
• Educate patients and their families about disease processes, treatment regimens, medications, and preventive strategies.
• Accurately document all care and patient interactions using Home Care Home Base (HCHB).
• Collaborate regularly with interdisciplinary team members such as therapy staff and medical social workers for coordinated care.
• Attend case conferences, team meetings, and required continuing education sessions.
• Maintain high standards of compliance with regulatory and agency guidelines.

Qualifications:

• Active RN license in good standing in the state of practice.
• Minimum of one year of recent home health experience (field-based preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, valid drivers license, and current auto insurance necessary.

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
