Job Description

Provide professional nursing care in a home health setting serving individuals in Green and Taylor counties, primarily based out of Campbellsville, KY. This contract RN role involves clinical assessment, care plan implementation, and health maintenance to support patients in their community.

Responsibilities:

  • Conduct thorough nursing assessments for well, chronic, or acutely ill individuals.
  • Implement and evaluate individualized care plans in collaboration with healthcare providers.
  • Monitor medication administration, ensure physician orders are transcribed and followed, and manage medication policies.
  • Document patient health status in nurses notes and complete required nursing reports and forms.
  • Coordinate with interdisciplinary teams to address patient health and safety needs.
  • Supervise and train licensed and unlicensed nursing staff to ensure quality care delivery.
  • Facilitate scheduling and follow-up for health-related appointments.
  • Maintain communication with hospitals during patient admissions and discharges.

Qualifications:

  • Current RN license required.
  • Strong clinical skills; home health experience preferred.
  • Valid driver's license with mileage reimbursement at $0.49/mile.
  • Ability to cover Green and Taylor counties, with primary coverage in Greensburg and Campbellsville.
  • Competent in supervising and training nursing personnel.

Schedule:

  • Monday through Friday, 8:00 a.m. to 5:00 p.m.
  • On-call every third weekend plus selected weekdays.
