Deliver compassionate, high-quality care in a field-based Home Health environment, supporting patients through personalized treatment in their homes. This full-time, 13-week contract position offers an engaging schedule and the opportunity to make a real difference for individuals in your community.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Duties:

• Conduct comprehensive OASIS assessments and formulate individualized care plans.
• Provide skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, assess any changes in condition, and communicate promptly with the care team and physicians.
• Educate patients and families on conditions, treatment options, medications, and prevention strategies.
• Accurately document care and assessments in Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for integrated patient care.
• Attend case conferences, participate in staff meetings, and engage in ongoing education.
• Ensure compliance with all regulatory and agency standards for documentation and care delivery.

Requirements:

• Active RN License in the state of practice, in good standing
• At least one year of recent home health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB)
• Experience completing OASIS assessments
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
