Take your nursing career into the community with an impactful 13-week full-time contract opportunity providing home health services. This field-based role delivers direct patient care and education in a dynamic setting, giving you the flexibility and autonomy to manage your own schedule while building connections with patients in their own homes.
Schedule and hours:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Expected caseload: 30 points per week (typically 35 patient visits per day)
Qualifications:
- Active RN license in the state of practice, in good standing
- Minimum of 1 year of recent home health experience (field-based experience preferred)
- Proficient in Home Care Home Base (HCHB)
- Comprehensive experience with OASIS assessments
- Reliable transportation, valid drivers license, and current auto insurance
Key Responsibilities:
- Complete thorough OASIS assessments and craft personalized care plans for each patient
- Deliver skilled nursing carewound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, promptly identify changes, and coordinate with care teams and physicians
- Educate patients and families on conditions, medications, and health management strategies
- Accurately document care using HCHB in a timely manner
- Work closely with therapists, social workers, and home health aides to ensure coordinated care
- Actively participate in staff meetings, case conferences, and ongoing education sessions
- Uphold all regulatory and agency standards for patient care and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
