Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with strong OASIS experience for a 13-week full-time home health contract. Schedule:
- 13-week contract with strong possibility for extension
- Full-time, Monday through Friday
- Expectation of structured daily patient visits
- Likely on-call rotation
Key Responsibilities:
- Deliver in-home skilled nursing care, including wound care, medication management, patient education, and chronic disease monitoring.
- Accurately complete patient assessments and timely OASIS documentation.
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans.
- Maintain proactive communication with clinical teams regarding changes in patient condition.
- Uphold professional standards for patient safety and care quality.
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation
- HomeCare HomeBase proficiency strongly preferred
- Ability to manage a consistent full-time visit schedule
- Valid drivers license and reliable transportation
