We are seeking a skilled Registered Nurse (RN) with a strong background in home health and expertise in OASIS for a 13-week full-time contract assignment. Schedule:
- 13-week contract, Monday through Friday (full-time hours)
- On-call duties likely as part of the weekly schedule
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions to assigned patients
- Conduct thorough assessments and document findings with precision using OASIS
- Collaborate with physicians and multidisciplinary care teams to develop and implement individualized care plans
- Carry out wound care, medication administration, disease management, and patient education
- Maintain effective communication with the clinical team regarding patient status and care needs
Qualifications:
- Valid and active RN license
- At least one year of recent experience working as a home health RN
- Prior OASIS documentation experience is essential
- Proficiency in HomeCare HomeBase is highly preferred
- Ability to efficiently organize daily patient visits within a structured schedule
- Dependable transportation and a valid drivers license
