A full-time registered nurse position specializing in home health is available for a 13-week contract, offering a structured work schedule and the chance to make a real impact on patients lives. Schedule:
- Full-time: Monday through Friday
- On-call responsibilities are probable
- Structured daily visit expectations
Key Responsibilities:
- Deliver quality in-home nursing care and skilled clinical interventions
- Complete timely and accurate patient assessments and OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to design and implement individualized care plans
- Administer wound care, oversee medication regimens, provide disease monitoring, and educate patients and families
- Clearly communicate patient status updates and changes in condition to the clinical team
Who You Are:
- Hold an active RN license
- Possess a minimum of one year of recent home health RN experience
- Demonstrate proven experience with OASIS documentation
- Proficient in using HomeCare HomeBase (strongly preferred)
- Comfortable managing a structured, full-time schedule with daily patient visits
- Have reliable transportation and a valid drivers license
