We're seeking a skilled RN with strong OASIS experience for a 13-week full-time contract. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Required Qualifications:
- Active Registered Nurse license
- At least one year of recent home health RN experience
- Proven proficiency with OASIS documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Ability to manage daily patient visits with a structured schedule
Key Responsibilities:
- Deliver in-home nursing care, including skilled clinical interventions and assessments
- Complete OASIS documentation with accuracy and timeliness
- Coordinate with physicians and multidisciplinary teams to create individualized patient care plans
- Provide education, medication management, wound care, and monitor disease progression
- Communicate changes in patient status promptly to the care team
