Delivering compassionate care directly to patients' homes, this contract position offers a fulfilling opportunity to support individuals across the lifespanfrom newborns to seniorsin the Cambridge, MA area. As a registered nurse specializing in home health, you play a crucial role in individualized care and patient support, with a strong focus on clinical assessment, care planning, and holistic education.
Schedule/Hours:
- Willingness to work weekends, holidays, and take on-call shifts based on departmental rotation
- Weekend rotation occurs once every six weeks
Qualifications:
- Active and unrestricted RN licensure in Massachusetts
- Minimum one year of certified home care nursing experience
- At least one year of case management experience
- Reliable, independent transportation for travel to patients' residences
- Commitment to working weekends, holidays, and on-call shifts as required
Key Responsibilities:
- Deliver direct clinical services to patients of all ages within their homes
- Conduct comprehensive patient assessments and tailor nursing care plans accordingly
- Guide, educate, and empower patients and their families on condition-specific care and health maintenance
- Complete the admissions process and case management in line with industry regulations and established standards of care
- Maintain accurate, thorough electronic documentation and support interdisciplinary collaboration for optimal patient outcomes
