Deliver skilled nursing care in home health settings, supporting patients across all age groups from newborns to centenarians. This contract role offers the opportunity to provide comprehensive clinical services, including patient assessments, care plan implementation, and education tailored to individual and family needs.

Key Responsibilities:
- Conduct thorough patient assessments and implement personalized care plans in patients homes.
- Provide ongoing education and support to patients and families to promote health and wellness.
- Manage admissions and case management duties following established home care policies.
- Maintain detailed, timely documentation through electronic health records.
- Collaborate with interdisciplinary teams to coordinate effective patient care.
- Participate in weekend rotations approximately once every six weeks, including holidays and on-call duties as required.
Qualifications:
- Current Registered Nurse licensure in Massachusetts.
- Minimum 1 year of certified home care nursing experience.
- At least 1 year experience in case management.
- Reliable, independent transportation to visit patients in their homes.
- Flexibility to work weekends, holidays, and on-call shifts per departmental schedule.
