Job Description

Deliver critical care and support as a Registered Nurse in a home health setting, serving patients with professionalism and compassion. This contract opportunity offers the chance to work independently within a clinical framework, applying nursing expertise directly where its needed mostpatients homes.

Schedule/Hours:

  • Weekend rotations are required once every 6 weeks.

Qualifications:

  • Must hold a current and unrestricted RN license.
  • Minimum of 1 year of certified home care nursing experience is required.
  • At least 1 year of case management experience in a clinical environment.
  • Proficient in completing admission processes and developing individualized care plans.
  • Reliable transportation and willingness to travel to assigned cases within the service area.
  • Strong skills in electronic documentation and interdisciplinary communication.
  • Commitment to delivering care aligned with current policy, standards, and best practices in community health.

Key Responsibilities:

  • Assess patient conditions and identify evolving healthcare needs.
  • Implement and evaluate individualized care plans, in consultation with patients and interdisciplinary teams.
  • Carry out comprehensive admission assessments and ongoing case management duties.
  • Provide patient and family education, support, and resources for successful care at home.
  • Maintain complete, timely, and accurate clinical documentation.
  • Communicate effectively with teammates and other healthcare providers to ensure continuity and quality of care.
  • Adhere to all organizational protocols for quality, safety, and compliance.
