Embark on a rewarding, full-time opportunity as a Registered Nurse in a field-based home health setting for a 13-week contract. Deliver high-quality patient care in the comfort of clients homes while enjoying a predictable daytime schedule.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience (field work strongly preferred)
• Comprehensive understanding and practical experience with OASIS assessments
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Reliable vehicle, current drivers license, and valid auto insurance (required for daily travel within territory)

Key Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes in condition, and communicate updates to the care team and physicians promptly
• Educate patients and their families about disease management, treatment regimens, and preventive strategies
• Accurately document all assessments, interventions, and patient outcomes using HCHB
• Coordinate with physical therapists, occupational therapists, social workers, and home health aides for integrated patient support
• Participate in team meetings, case conferences, and required continuing education sessions
• Maintain compliance with agency and regulatory standards for in-home patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
