Your next nursing opportunity begins herea full-time, 13-week contract for a Registered Nurse skilled in home health and OASIS documentation. This role is perfect for clinicians who excel in providing patient-centered care, thrive in autonomous environments, and possess a strong attention to detail.
Schedule:
- Full-time assignment, Monday through Friday
- On-call participation may be required
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes, ensuring high-quality clinical outcomes
- Complete thorough patient assessments and submit accurate OASIS documentation within required timeframes
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Perform wound care, medication administration, patient and caregiver education, and chronic disease management
- Maintain clear, timely communication with the clinical team about status changes or patient needs
Preferred Experience & Qualifications:
- Current, active Registered Nurse license
- Minimum of one year recent home health RN experience
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Excellent time management skills to handle a structured daily visit schedule
- Reliable transportation and a valid drivers license
