Experienced Registered Nurses (RN) with strong home health backgrounds and OASIS expertise are needed for a full-time, 13-week home health contract in Missouri. This role places you at the forefront of patient care, offering autonomy and the chance to truly impact lives in a home setting.
Schedule:
- Full-time assignment, Monday through Friday
- On-call availability may be required
- Structured daily visit schedule
Key Responsibilities:
- Deliver comprehensive skilled nursing care to patients in their homes
- Perform thorough patient assessments with accurate, timely OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to implement individualized plans
- Administer wound care, medication management, patient education, and ongoing disease monitoring
- Report change in patient conditions and progress clearly to clinical leadership
Qualifications:
- Current and active RN license
- At least 1 year of recent RN experience in home health settings
- Proven OASIS documentation experience required
- Demonstrated proficiency with HomeCare HomeBase preferred
- Capable of managing daily patient visits within a structured, full-time framework
- Reliable transportation and a valid drivers license
