Your next nursing opportunity begins here: We are seeking an experienced and compassionate Registered Nurse (RN) with strong OASIS skills to provide home health care services on a 13-week full-time contract. If you excel in clinical practice, meticulous documentation, and committed patient management, this role offers a chance to make a genuine impact while working with a collaborative care team.

Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required

Key Responsibilities:

• Deliver skilled in-home nursing care and interventions for a diverse patient population
• Complete patient assessments and accurate OASIS documentation in a timely manner
• Collaborate with physicians and multidisciplinary care teams to develop and follow individualized care plans
• Administer wound care, manage medications, conduct patient and caregiver education, and monitor ongoing disease processes
• Communicate promptly with the clinical team about patient condition updates or changes

Qualifications:

• Active RN license
• Minimum 1 year of recent home health RN experience
• Proven OASIS assessment and documentation skills
• Proficiency with HomeCare HomeBase (strongly preferred)
• Ability to effectively manage full-time schedules with daily visit targets
• Reliable transportation and valid drivers license
