This 13-week, full-time contract role centers on delivering quality care and education directly to patients in their homes, allowing for a personal approach that truly impacts recovery and well-being.

Schedule & Expectations:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based; daily travel within assigned territory (mileage reimbursement provided)
• Caseload: Manage 30 points weekly (typically 35 patient visits per day)

What Youll Do:

• Conduct comprehensive assessments using OASIS and develop individualized care plans for each patient
• Deliver skilled nursing care, from wound care and medication administration to IV therapy and chronic disease management
• Continuously monitor patient progress, identifying and reporting any changes in condition to the care team and physicians
• Provide education to patients and families regarding disease processes, medications, treatment plans, and preventive care
• Complete accurate, timely documentation in Home Care Home Base (HCHB)
• Collaborate effectively with physical/occupational therapists, medical social workers, and home health aides to ensure coordinated, holistic care
• Participate in case conferences, staff meetings, and ongoing professional development as needed
• Uphold rigorous standards for documentation and patient care per regulatory and agency guidelines

Required Qualifications:

• Active and unrestricted RN License in the state of practice
• At least 1 year of recent home health nursing experience, ideally with field experience
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
