Job Description

Deliver expert care in a rewarding field-based position, supporting patients in their homes. This full-time, 13-week contract offers a MondayFriday schedule from 8:00 AM to 5:00 PM, with a manageable caseload averaging 35 patient visits each day (30 points per week). Youll travel throughout your assigned territory providing critical nursing services, supported by mileage reimbursement and a robust benefits package.

Qualifications:

  • Active Registered Nurse (RN) license in good standing for the state of practice
  • Minimum 1 year of recent Home Health nursing experience (field preferred)
  • Proficiency in Home Care Home Base (HCHB) electronic documentation system
  • Demonstrated experience with OASIS assessments
  • Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop personalized care plans
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, identify changes, and quickly communicate with care teams and physicians
  • Provide patient and family education on diagnoses, medications, and health maintenance
  • Maintain detailed, timely documentation using Home Care Home Base (HCHB)
  • Work collaboratively with multidisciplinary teams including therapists, social workers, and home health aides
  • Join in case conferences, meetings, and ongoing training to ensure top-tier care
  • Stay compliant with all regulatory and agency standards

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
