Advance your nursing career in a dynamic home health environment with a full-time 13-week contract opportunity. This field-based role allows you to make a meaningful impact by delivering skilled care directly to patients in their homes. Work Monday through Friday, 8:00 AM to 5:00 PM, managing a caseload that averages 35 home visits daily, with an expectation of 30 points per week.

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans.
• Administer skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress and communicate changes to the interdisciplinary care team and physicians.
• Provide education to patients and families on treatment plans, medications, and preventive health.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for seamless patient care.
• Participate in regular staff meetings, case conferences, and professional development.
• Ensure compliance with all regulatory and agency policies.

Required Qualifications:

• Current RN license in good standing for the state of practice.
• At least one year of recent field-based home health nursing experience.
• Experience with Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, valid drivers license, and active auto insurance.

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
