Bring your passion for nursing and community-based care to a dynamic, field-focused role supporting patients in their homes. As a Registered Nurse specializing in Home Health, you'll work independently to deliver skilled nursing services while collaborating with a multidisciplinary teamall with the support and resources you need to succeed.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Your Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized care plans for each patient.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management in the home setting.
• Monitor and assess patient progress, promptly identifying and reporting changes in condition to the interdisciplinary care team and physicians.
• Educate patients and families about disease management, medication regimens, and preventive health strategies.
• Accurately document care and assessments using Home Care Home Base (HCHB).
• Collaborate closely with therapists, social workers, and aides to ensure coordinated, holistic care.
• Attend case conferences and staff meetings, contributing your expertise to ongoing care planning.
• Uphold the highest standards for documentation and patient care, in full compliance with regulatory and agency guidelines.
• Travel daily within the assigned territory (mileage reimbursement provided).

Qualifications:

• Active RN license in the state of practice, in good standing
• At least 1 year of recent Home Health nursing experience (field work preferred)
• Strong proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Reliable transportation, valid drivers license, and auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
