A fulfilling opportunity awaits for an experienced Registered Nurse to provide compassionate care to patients in their homes. This full-time, 13-week contract position focuses on supporting individuals in the community through skilled nursing care, comprehensive assessments, and personalized interventions in a field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans tailored to each patients needs.
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Perform regular evaluations to monitor patient progress, promptly report changes in condition, and communicate with care teams and physicians.
• Educate patients and their families about conditions, treatment plans, medications, and preventive care.
• Accurately document all care and patient interactions using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team memberssuch as therapists, social workers, and home health aidesto provide coordinated care.
• Participate in case conferences, staff meetings, and ongoing professional development activities.
• Ensure compliance with all documentation, regulatory, and patient care standards.
• Complete daily travel within the assigned territory; mileage reimbursement is provided.

Qualifications:

• Active RN license in good standing in the state of practice.
• Minimum 1 year of recent home health nursing experience; field experience strongly preferred.
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS.
• Reliable transportation, valid drivers license, and auto insurance.

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
